WARSAW — A Milford man died after a two-vehicle crash north of Warsaw Saturday night.

At 5:35 p.m. on Saturday, June 29, Kosciusko County Sheriff's Office deputies and emergency personnel responded to a crash on CR 150W, south of CR 400N.

The preliminary crash investigation determined that a 2024 Tesla driven by Anish Daita, 31, of Warsaw, was traveling north on CR 150W when it veered left of center, striking a 2007 Chevrolet driven by Joseph L. Criswell, 44, of Milford. Criswell was traveling south on CR 150W when his vehicle was struck head-on by Daita's.

Criswell was pronounced dead at the scene by the Kosciusko County Coroner's Office from injuries sustained in the crash. Six other passengers in the Chevrolet, all under the age of 18, were treated for various injuries. Two girls were flown to Lutheran Hospital in Fort Wayne for treatment. One girl was transported by ground ambulance to Lutheran Hospital and two boys were treated at the scene for minor injuries.

Daita suffered internal injuries and was transported to Lutheran Hospital in Fort Wayne. Two other adult passengers in the Tesla were treated and released at the scene for minor lacerations and bruises.

KCSO was assisted at the scene by the Warsaw Police Department, Plain Township Fire Department, Lutheran Ground and Air Ambulance, Kosciusko County Coroner's Office, North Webster EMS and Crouse Wrecker Service.

The accident remains under investigation by the KCSO FACT Team.
